Outpatient Therapy and Home Health Partners

A practical, ethical option for patients who are “not yet” ready for skilled care

Safer to Stay partners with therapy practices and home health agencies to reduce fall risk, support appropriate utilization, and preserve trust with patients and families. We focus on the “not yet” group—those who have real risk but don’t yet meet criteria, are hesitant, or are declining services.

How We Fit Into Your Care Plan

How Safer to Stay Supports Your Team
  • Provides neutral, non-medical home safety movement assessments for patients who are not ready or eligible for skilled services
  • Gives families concrete, step‑by‑step guidance to reduce fall risk at home
  • Helps clarify when it’s time to re‑approach therapy or home health with stronger readiness and buy‑in
  • Offers clear documentation you can reference in your own notes and conversations

When to Refer to Safer to Stay

When to Refer a Patient
  • A patient or family is worried about falls, but doesn’t yet meet criteria for skilled services
  • A patient declines therapy or home health, but you’re still concerned about safety
  • You’re sensing a pattern of “near misses” at home that could turn into a fall
  • You want an objective, non‑medical voice to reinforce the safety message
  • You need an ethical option instead of stretching eligibility or documentation

For Outpatient Therapy Clinics

Supporting Patient Safety Beyond the Clinic

Outpatient therapy plays a critical role in helping individuals improve strength, balance, and mobility. However, many safety challenges occur within the home environment, where therapists often do not have the opportunity to observe patients navigating their daily routines. Safer to Stay helps extend the clinical perspective into the home.

Through in-home safety and movement assessments led by a Doctor of Physical Therapy, Safer to Stay evaluates how individuals move through their own environment — including walking pathways, stair navigation, bathroom access, lighting, and everyday transitions. This real-world observation can uncover environmental factors that may contribute to falls or mobility challenges.

How Safer to Stay Can Support Your Patients
When a Referral May Be Helpful
A Collaborative Approach
  • Observation of real-world movement within the home environment
  • Identification of environmental fall risks
  • Practical safety recommendations to support mobility
  • Guidance for individuals planning to age safely at home
  • Objective feedback for families concerned about home safety
  • A patient has experienced recent falls or near-falls
  • There is uncertainty about safety at home
  • A patient plans to age in place long-term
  • Family members are expressing increasing safety concerns
  • A patient is approaching discharge but environmental concerns remain

Safer to Stay is designed to complement, not replace, therapy services. With patient consent, Safer to Stay can share general safety observations and recommendations that may help therapy teams better understand the environment their patients navigate each day. Together, the goal is to help individuals maintain independence, reduce fall risk, and remain safe in the homes they value.

A home safety assessment can provide additional clarity and support during these situations.
